Isekai manga following non-attractive main character?

The more unattractive the better, especially if they don’t have a training arc to get hotter. Kirito-lookalikes blaming their ”mediocre looks” for their lack of popularity don’t count. Bonus points if they’re treated as lesser than for their ugliness.

MC has a uncle that was in coma for years. This uncle had Isekaied and the MC didn't know.

Sometimes the uncle tell about his adventures on fantasy world and almost everytime he gets screwed for being ugly.

Yup this is the best one, I love that part where he removes the mask and the human woman just goes "wtf you are soo ugly are you a half-breed of a human and a goblin or something, so that's why you wear a mask, disgusting" and what happens to her after that, his personality is also super interesting and the op is amazing

Tanaka - Nenrei Equal Kanojo Inaireki no Mahou Tsukai in novel format, or Tanaka the Wizard or Tanaka no Atelier in manga format (though both got axed), has the MC as an ugly guy that gets a hard time most everywhere he goes. At least one person manages to look past his ugliness and like him for... reasons.

There's another sort of piggy prince kind of story where he tries and mostly fails to lose weight, but I can't remember it at the moment.

Yeah, maybe. In Piggy Duke, I know he kind of starts to lose weight toward the end of the manga run, but I never read the novel so can't speak on that. But it's a strangely common trope. Solo Bug Player is another where he starts out extremely fat but I think he loses it pretty quick.

Tsuki ga Michibiku Isekai Douchuu, he is apparently so ugly that when he met the goddess of the world he was supposed to be Isekaid into, she cursed him to not be able to speak human language and banished him to a place where only demihumans exist.

A guy gets isekaied but before that he gets a chance to modify his stats. The more disadvantages you take on, the more points/power perks you can get. So the guy sets his looks to -255 (as a point of pride over his real-life lack of good looks) and places on himself a curse which drains his exp and causes him severe pain if he comes in contact with a woman/girl. Gets a huge bunch of points to put into stats but could only apply part of it before time runs out and off he goes.

Edit: Oh f-k, I'm so very very very sorry. I just assumed there was an English scanlation when I saw the title on mangakakalot. Or at least that it came out in English somewhere. I'm an a-hole for not properly checking. I SUCK.😫

“I Got a Cheat Skill in Another World and Became Unrivaled in The Real World, Too” is great though it doesn’t match exactly as you’re describing, as the character becomes conventionally attractive pretty early on.

It’s about a high schooler who is bullied relentlessly by his family and peers for his appearance and weight, only having been treated kindly by his late grandfather. His grandfather taught him that kindness was the most important trait, however, and ended up willing his home and belongings to the MC- which included a hidden room with a portal to another world. His leveling in the fantasy world changes his appearance in the real world, where people suddenly care about him and see him as handsome. He still has the same mindset, though, and stays humble and kind while not quite understanding/believing everyone’s love for him and his new face. It’s a fun anime, with a great balance between his real life and his fantasy adventures. (Also, the main love interest liked the MC before he was hot if that makes things better :))

Saikyou Yuusha wa Oharai Hako. The main character isnt really ugly in my opinion but he is by the world's because he is a fat and has a l flat nose. He doesn't change physically but emotionally as the people he becomes allies with don't see him as ugly or fat

"Creature Girls - A Hands-On Field Journal in Another World" there is no possible way for me to describe the 2 main plots of this story in a simple comment without spending the next two days transcribing the entire manga. Just trust me and read it, the second plot is exactly what you described. A character that not only is ugly, but also deeply insecure about his unatractiveness. While everyone tries to treat him in the best way possible because he has modern knowledge, they can't fully hide their loathing for him and it becomes the central driving force of his entire character arc. Do note that this is the B plot, so while you get hints about what is going one thru the early chapters, he only gets actual full chapters starting at chapter 22.
