Never done a deriheru before and have some questions. And yes, I can speak Japanese fine.

I understand that there's an option for online booking for some places, but I'm assuming calling is better?

What is the process like on the phone? What type of questions are asked? Do they ask about amenities and such? And finally, when the reservation is made is the process for getting into the hotel, calling, then receiving a call later all the same?

Depends entirely on the shop. Some are quick, some never check. Calling is always better. Info on City Heaven is not always accurate (particularly availability.)

City Heaven is just a communication portal for all kinds of different shops with different policies. When you "reserve" on CH you are just taking the first step. The shop will contact you as to what their next steps are. Either by phone or CH message or whatever is usual for them. Shops are all different as to their procedures - both for the reservation and for the day of your meeting. If you speak Japanese you will have no issue. And no need to stress about any of this. Just go for it bro.

I don't get appointments on CH I just call the shop and ask if gaijin is ok. Usually they'd reply well it depends on the girl. And you say how about this girl. And the tencho is like nah she doesn't accept, then you say how about this girl ... this goes on and on. Once he accepts, you say okay, is she open at 6pm? they say yeah, you say ok 6ji kara de onegaishimasu. then he'll say once you get close call or something. Or get to the rental room and call the room no. or something. All this depends on the shop and none of this might happen.

One way or another the shop will want to know what kind of man they are dealing with. If you can speak Japanese with no issues, it should be smooth sailing; they will be forefront and tell you what is okay and what isn't. If you struggle to speak Japanese well, some people get impatient. Or even worse, if you make a mistake, they will just full on refuse you.

Everyone once in a while you'll even find a shop where everything is so easy, you can be booked and ready to get health delivered as fast as 20 minutes later (especially if you already got your room and are determined to get a girl in there.)

Cityheaven reservation is just "an entrance" to the booking process. The store always will call you immediately as the get the reservation request.
Most Deriheru will accept your reservation as long as your Japanese is average (daily life conversation). But be aware that some girls do not accept foreigners even if they are fluent in Japanese.
Soaplands are more strict regarding their no foreigners policy. But of course, the store will inform you immediately after they call you. Some do it politely others in a very rude way, but they will let you know for sure.
But keep in mind that they will call you to confirm your reservation.
