Depends what you mean by real. In general, if the site shows their unblurred faces, it's probably photoshopped to some extent. And sometimes those body shots are 'real' but 10 years and 15 pounds ago too.

If you want to be sure what you see is what you get, then you're limited to those girls who have blogs/SNS linked to their profiles. And even then, with Snapchat and such able to change appearances, YMMV. But it's pretty obvious when they use filters on their pics.

Though not fool proof, IMO your best bet is to just look at measurements and diary pics, and find out whether whatever you are looking for is shown in the diary pics. For example, if the profile says she has huge tits or a small waist, but it's not shown in her diary pics, it may not be accurate. Also, if she uploads the same cycle of pics over and over for a long period of time, they may be out of date.

Like Acid Burn said above, it depends on what you mean by real. I usually use shops with reviews, I've blindly booked an unreviewed shop once, but I've never had someone show up that looked drastically different than their profile. You can do online reservation (net yoyaku) but they will call you, at least the first time.
