Breast cancer is present in transgender men and the risk is dependent on top surgery; those with top surgery appear to be lower risk than natal females. More longitudinal studies and better population data are required to contribute to evidence-based screening recommendations.

The data collected shows a younger age at diagnosis, with a FtM median age of 44.5 years versus natal female median age of 62 years. Interestingly, of the nine recorded presentations, four FtM patients presented with local-regional or metastatic disease (44.4%); this may represent aggressive cancer, as breast cancers diagnosed at a younger age in natal women tend to be more aggressive.

In the setting of top surgery for transgender men, a surgeon may focus on the aesthetics of a masculine chest which may leave residual breast tissue behind. For any patient, whether a transgender man or natal female, breast tissue may not be entirely removed with mastectomy, especially with preservation of the nipple and areolar complex and this remains an essential component of patient education.

Current recommendations for transgender men who have not had chest surgery suggest screening as cisgender women. Other guidelines specify transgender men over the age of 50 who have not had chest surgery should undergo annual chest/breast exams and regular screening mammography regardless of CSH.
