Body parts include:
- head (hair)
- face (mouth/lips/cheeks)
- arms (hands)
- chest (body/wings)
There is no difference between left or right body parts.
Big heart actions are actions that gives 40 intimacy bonus points. They are marked with 💕.
Normal heart actions are actions that gives 20 intimacy bonus points.
Gifts are given after getting heart reactions for all three interactions. Gifts are only given 6 times per day (two each of AP x10, Grimm x1,000, Raven x1).
On the day of the Player's Birthday, every interaction (with the exception of item gifting) will result in big hearts.

Is it just me or does this happen to anyone else?

Sometimes, when I do ignore -> face rub -> head rub, I get small hearts, sparkles, and sparkles, respectively. Normally, if I do this exact interaction, I would get small hearts, big hearts, and big hearts, so I don't know what I'm doing wrong whenever I get sparkles.
I personally have not had any issues. I tried again today as well with the sequence you did (ignore -> face rub -> head rub). It could be counting your face rub as a tap? Not sure, sometimes the game is just weird.
When I rub his face I make sure to rub from the bottom of his chin up to about the bottom of his ear, up and down. Seems to be quite consistent for me in that area.
I don't know. It was really weird for me. The first time I had this issue, I was in a car, so I thought I shook my phone by accident. (Checking the interaction chart today, my assumption was incorrect because I would probably have hearts instead if I actually shook my phone.) I don't think the rub counted as a tap because when I looked at the chart, face tap on the second interaction results in small hearts, and head tap results in small hearts. (Maybe the chart is inaccurate? I'm not sure.)
I'll try your way the next time I have the "The Young Master will be glad to see you are progressing with your lessons." interaction.

I know for sure I have had the same issue where I do the sequence correctly and it just decides one of them is no good. Not sure if it's lag or what but it sucks since you need so many points to get to the higher levels of affection.

His 1/2 star sequence is completely wrong. He says "I am simply glad I could be of service" and he doesn't like any of those interactions at all. I've never gotten reactions so negative

I think the "Is there some way I may be of service?" and "I am simply glad I could be of service." quotes are swapped because I got the "Is there some way I may be of service?" quote as my home interaction quote once.
I don't know how to edit my own comment, but here is some stuff.

Giving him Madam Scream's Macarons and then rubbing his face will lead to perfect hearts and small hearts, respectively. (I don't know why rubbing his face on the second interaction after giving him an item will change the reaction.)
"Please continue as you are now, striving to do your best."

For the "Please continue as you are now, striving to do your best." do shake, arm tap (right arm), and head rub (mega hearts)
For Battle with 3 stars, do Highfive --> Tap Chest --> Tap Chest for Big Hearts (Tap chest gives big hearts both times). The line is "High five? Is this sufficient? "
You can also do Head tap --> Head Tap --> Head Tap for little hearts if its not a highfive.
In a battle with 3 stars. When he says, "Please continue as you are now, striving to do your best." Do a Head Tap, Head Tap and then a Head Rub. In the last interaction you will get a lot of hearts.
When he says "The young Master will be glad to see you are progressing with your lessons." You can tap his arm in 3rd action and you will get one simple heart
