My monetization is disable and its showing "Monetization on this account has been disabled due to repeated submission of ineligible videos and/or insufficient documentation"
Pls guide me what to do?

You should have received an email and a notification with more info in Creator Studio. Take a look at those resources and note that you can always reapply for the YouTube Parter Program again in 30 days.
I Didn't received any mail or notification for youtube. I have crossed 1k subscriber & 4000 watch hr in april itself. & my monetization is disable from dec 17 & this message is showing around 2 to 3 months .. there is no option to re-apply also.
