I don't remember this practice in early manuscripts, but I haven't studied all the manuscripts :-) . Perhaps in mss written in D script, ?

For that practice however, we find many examples in the early manuscripts. Perhaps the /a/ vowel was added for securing the reading and avoiding the risk to read with imâla

No, I've never seen superscript alif in early manuscript before full alif. That is something I mostly associate with Ottoman Qurans. This is the earliest manuscript where I see it in practice.
The use of red miniature alif in words like ذلك with defective ā is common though.
