Over the last week, & I travelled around Maharashtra, visiting ten public, private, urban and rural hospitals - meeting ENT doctors, mucormycosis patients and their families
It was phenomenally difficult but we hope it will lead to changes in detection & treatment

Doctors across Maharashtra all agree that we are seeing a massive, massive surge in mucormycosis cases since mid-March - that we crucially did not see during the first wave.
There were also just 41 post-Covid mucor cases globally by March 1
So, what has changed?

The surge was initially blamed on over-prescription and long-term use of steroids.
While doctors say steroids do suppress the immune system, they argue it is incorrect to blame this sudden explosion in mucor cases on this factor alone.
Why?

Steroid overuse has been endemic in India for a long time. If this was the cause, doctors would see large numbers of mucor cases in other patients. They aren't.
Steroids were also prescribed en masse during the first wave - we didn't see this huge mucor surge then, either.

“We need more investigation into this but we are seeing this concerning new pandemic, around 40 per cent more patients are developing diabetes after having Covid-19 during this second wave,” said Dr Kothalkar - some with blood glucose levels as high as 700mg/dL

This is another key point to make. There is a lot of fear around mucor but if we detect it early, it can be treated very effectively.
If we wait, even a few days, the fatality rate can be as high as 50-80%
Symptoms: discharge from the nose, headache, blurred sight, toothache

There is already research to suggest a link between Covid and diabetes.
Studies in the British Medical Journal and Nature found 5-14% of hospitalised Covid patients developed diabetes
Doctors here strongly believe the Delta variant is now causing this to happen more regularly

The key takeaway of our week in the field - doctors say we need to study the link between "Delta", post-Covid diabetes & pancreatic damage, and mucormycosis to understand the link

There's so much else I would like to write but this thread is on the long side already
You can read (all pics used here are Simons and shared with consent) & I's long-read from our week on Maharashtra's mucormycosis wards here for free

thank you. the advisory needs to go out to get blood sugar levels monitored even after vacc. my fathers sugar went out of whack after vacc. we dismissed it as unrelated but a doc last wk alluded to what you wrote about a link between covid and pancreas.

Thanks for really informative thread.
One question though: There is huge difference ( order of magnitude of 100 times) in Mucormycosis rate between Kerala on one hand and Gujarat and Maharashtra on other hand. What can be the possible reason?

This is called "clickbait" and 𝗳𝗲𝗮𝗿-𝗺𝗼𝗻𝗴𝗲𝗿𝗶𝗻𝗴 is the ONLY way to get "likes"; so they take the outliers (5% of hospitalized, not ALL infected) and blow it up to create fear and eventually MORE "likes" on twitter

We need to perform statistical analysis to reach better conclusion. Of all those patients having blood sugar, how many of them had taken steroids? Or antibiotics or any other medicine? And how many can be explained away as not due to COVID but some pre-existing medical condition?

Thank you for doing this critical survey at this critical juncture and sharing it here.
very informative!

Informative but scary thread…Covid 19 and it’s variants just don’t seem to relent.
