Summary:

A yellow butterflyfish with dark scales margins that appear cross-hatched or lattice-like, a black bar through the eye, a blue forehead, a dark submarginal band on the soft dorsal fin and a broad black bar through the middle of the caudal fin. 

Some individuals have a black spot below the dorsal fin  spines. Juveniles have a dark spot on the soft dorsal fin.
