You have access to more than 4,300 providers, 12 hospitals and 650 clinics in the UCare Medicare with M Health Fairview & North Memorial Health network. We also include many independent providers, like Entira Family Clinics and Voyage Healthcare (formerly North Clinic).

Caregiver Assurance™

Members and caregivers are eligible for three phone consultations with a Caregiver Advisor – a licensed social worker trained in aging and caregiving. After your free consultations, your advisor will provide a tailored quick-step plan as well as guidance, resources, and service referral recommendations to help support your caregiving needs.
