   @Matt D. Smith - There are a lot of nested interactions happening all at once (i.e. tab menu > radio button selection > text form fields). It is also not clear whether the COD checkbox falls under Form of Payment or is a separate entity (since you have a drop down menu, and immediately after you have the check box). Finally, the language used throughout the form in general is not all that clear either. This might be clear in the client's context, but it is not for a user outside looking in.

   Visual design, however, is clean, neat, and well balanced. I love the UI style and how you adapted it from mobile to desktop.

   @Roberto You are correct, this form is for a very specific type of user who would be really familiar with the content. There's always room for improvement though - thanks for the comments!
