This repo is missing an issue template :)

• Fix gitlab template directory (gitlab => .gitlab)
• Add issue template

This seems like a trivial chore task I could create a PR for. I assume a good starting point would be to copy the issue template from other projects sans label macro?

Certainly would be that simple!

Oh, there's an existing gitlab folder but it's not named properly. I'll fix that.
