Puffy Vulva is a catchphrase associated to Spice and Wolf's character Holo, and generally associated with pictures of anime characters getting their cheeks pinched, also used as spam and shitposting practise.
Origin
The earliest use of the term can be found on 4chan's anime and manga discussion board /a/, in form of haiku related to Holo, the main character of the light novel and anime series Spice and Wolf who is a wolf under human appearance, submitted on February 3rd, 2008.[2] Although, is believed that the term started being used on Spice and Wolf's anime premiere, during January, 2008.
Horo is not a furry,
only her ears and her tail are,
and her hot puffy vulva

At some point, the term started being associated with an edited image of Holo pinching her cheeks. The original panel can be found on the fourth page of Spice and Wolf's manga's fourth chapter.
Monster Girls
On the monster girls fandom, the term "puffy vulva" has been associated to fur-like humanoid girls (like nekos or kitsunes) due Holo's wolfgirl appearence. This has inspired other terms to refer to other brands of monsters, like "snake cloaca"[3] for Lamias or "horse pussy"[4] for centaurs, the last one being also used by members of My Little Pony's fanbase
