Please be prepared to enter a prefered user_name and a password.
The user name should be between 6 and 18 characters long and should
contain only alphanumeric characters (plus '_' or '.'). Examples
of valid usernames:
Martel, martel, E.A.Martel, Ed_Martel.
All other fields may contain ISO Latin-1 characters.
Note: All entries are case-sensitive.

Please separate multiple phones by comma or semicolon and separate
an optional extension by "/".
