Someone from Setagaya-ku city office just called on my phone, switched to English mid-call and... I didn't notice, wondered why I couldn't understand his Japanese all of a sudden.
Had to ask him to repeat... in Japanese 
I always feel bad when someone is making a lot of effort in English but I can't understand and have to ask them to speak in Japanese. But... Not my fault if I'm not an native English speaker and can't understand other non-natives accents.
