Of course we all get old, fat, and frail.

The point is as a physician you will be much better when are 50. A female that only has her looks will be in a different position.

My dad is 71 and still gets around quiet easily as opposed to my uncle and his brother who at 73 can't walk without assistance from another person or his walker. One has heavily smoked and drank his whole life, the other hasn't. It all depends on how you take care of your body. However I do agree, men tend to hit "the wall" at a much later age and at a much slower rate than women.

Men physically peak at 31 give or take, but we can continue gaining strength into our early 40s. You will only be able to maintain/slowly lost strength after this.

Don't confuse physical strength with the the ability to fight/wrestle. While strength plays a role it is largely skill based. I have a buddy who is absolutely jacked, but I would prefer his sister in high school back me up in a fight. The man just doesn't know how to fight in any capacity.

Technically the wall doesn't exist for men, it's more like a slope. You can still reproduce and stay in good shape/strength well into older age. It just becomes more difficult to keep it up as time progresses.

I passed forty so long ago I can barely remember it, but I still lift and do a couple of very grueling outdoor sports. Guess what? My back doesn't hurt. My knees don't hurt. I get by just fine. The fatty twenty and thirty-somethings in the clubs I'm in? They get all tuckered out really fast on the uphills. Same with the sedentary 40+ ladies even if they're not that fat.

tl;dr Work out and keep your calories down somewhere around 2000 a day if you want to age well.
