That WILL happen IRL too at some point judging by how things develop now. People will be genetically "branded" at/before birth so that their body rejects off-brand organs or even off-brand medicine. Medical companies will earn gazillions and normal people will pay the price whether they like to or not since it's their only option.

Please explain how it is unlikely. The rest of the world is going this way, medical companies actively avoid curing things (it's a better profit to put a person at a lifetime subscription) and increase in wealth and power, doctors are regularly basically bought to issue certain medicines rather than actually trying to cure the patients, and so on.
The ethical step to what I say is really small, and money is really likely to win over ethics and morals in that battle. The only issue is that the technology doesn't currently exist.

You can't just genetically modify organs to be hyper selective with what they're compatible with. Also why would organs be branded if the chance to ever see them is like 2%

If the programming of our stem cells can be affected the right way it isn't beyond imagination that our entire body as a result can be coded into requiring a certain counter-part of DNA/whatever to be present in replacement organs.
We're just scraping the very surface of genetics at the moment in medicine, and there's still A LOT to explore and understand. It certainly wouldn't be unreasonable for someone to come up with a way to achieve this eventually.

I only know bio from an intro-level course, but I'm pretty sure the body naturally makes protein "markers" that differentiate your body from foreign organisms.
Try looking up some info on organ rejections. It has a lot to do with these kinds of things.
As far as your theories... I don't think we're even close to being able to manipulate tolerances to different medicines the way you describe. Just think about the fact that these medicines are mostly molecules composed just tens of atoms with very specific shapes. Those shapes and the tiny differences in charge distribution around the molecule are what make the medicine "effective" because it can bind to the proper receptors, or interrupt some process, etc.
And this is just me talking shit, the people who research pharmaceuticals every day have no idea how some medicines even work.
Your scenario may be possible very far in the future, but man I think it's not worth being concerned with it right now.
