Celebrity figures have show-biz names that can have strange and arbitrary orthography. Think of it more like a brand. This wrestler has determined that his wrestling name is KUSHIDA, so that's how they write it. The foreigner probably has no special presence in Japan so they use a standard transliteration that anyone will be able to read.

I neglected to tell them about an office move and a job change, and they realized it right after approving me for another three-year visa extension. When I went to pick up my new zairyū card they just told me to file the paperwork right after. There was no talk of penalties.

The only thing that might count as fallout is that I was rejected for PR, which I applied for at the same time as my visa extension, but I don't know if that's really the reason.
