I think he should look into CBT, or at least some kind of cognitive therapy, of the sort that helps people with PTSD and other issues where they have difficulty thinking about a subject rationally. Some exercises he can try himself are to think around particular women and why he doesn't see them as being the same, character-wise, as men. Bust through illogical reasoning and keep going. That's basically a very stripped-back DIY version of CBT, an actual psychologist would be able to help a lot more, but feel free to PM me if you want some more pointers/resources.

He's in CBT for a separate issue and he doesn't want to bring it up with his psychiatrist because she's a woman and doesn't want her to see him as a misogynist. He's not got long left so I doubt they could address it at this late a stage but if he goes back again I'll push him to try and raise the issue.

He should definitely bring it up. Psychiatrists, like doctors, actively do not judge their patients; it's not her concern why he has these opinions, only how to help him with them. The fact that he wants help means a lot. It's never too late for her to at least give him some tools to tackle it, so definitely tell him to bring it up!

If he finds it easier, he could frame it to her less like "I don't respect women" and more like "I met a girl and I'm having trouble respecting her, even though there's no reason not to".
