No, it's to mean poor people, those who would live in the ghetto. If you saw a black kid in the suburbs you would not say, "how ghetto." If you saw a black kid with gold chains and gold teeth with an unkempt style, you would say, "how ghetto." You would say the same thing if you saw a hispanic or caucasian with the same attire. In fact, the word, "ghetto," has arguably gotten less race oriented over the years, as it would only refer to ethnic minority communities historically. Of course, we do have a special word for poor white people, "trailer trash," but here is some cultural context needed when analyzing the difference. Poor white communities, usually in the rural south, are commonly, "trailer trash," whereas poor blacks usually live in urban areas. And those two have differences in attire, and it is no doubt the attire or behavior which we label ghetto, not the race, as we would clearly label this white guy, "ghetto."
