I will agree that it can vary greatly from provider to provider (some companies have policies that will cover anything, few questions asked). However, the trend (and, again, this was before ACA passed) was that Viagra was left to the insured to pay out of pocket (~$10-20 per pill, depending on dosage). In cases of heart issues, it wasn't uncommon that the insurance would still require prior authorization from the physician before they would cover it, just because of what drug it was (which is an excruciating process for all parties involved).

I do remember quitting right after a law passed that mandated some birth controls be covered 100% by insurance companies. Honestly? That was one of my fondest memories of the job. People were absolutely thrilled by this news and it changed the lives for some. I can only hope it becomes more comprehensive.
