Answered Windows Modules Installer has stopped working

    I have Windows Vista. Recently a window popped up on start up that says I need to re-install windows. It goes on to say that an unauthorized change was made to windows and it must be re-installed. If I had the discs, they have long since been misplaced. Is there any other way to reactivate windows? I've tried fixing it before, but never really got anywhere. Any help would be appreciated.

    You may find this funny, strange, or just unexplainable but here's what I found. I came up to my computer to start backing everything up onto an external hard drive. I noticed that my computer must have restarted itself because it was left at my login screen even though I know for a fact that it was logged in when I left. So I logged in as normal. When everything loaded up, there was no popup that said Windows module stopped working. There was no mention in the lower right corner that this was an unauthorized copy of Windows. There was no screen telling me to activate now or later. In other words, everything appears as normal. I don't know if there is a way to check it or if it is even worth it. But something you did fixed it and I am extremely grateful for it. Thank you very much.

     Fille Mismatch: C:\Windows\system32\ktmw32.dll[Hr = 0x80004005]

    is the source  of your probem.

    Please run a full CHKDSK and SFC scan....

    Click on Start > All Programs > Accessories

    Right-click on the Command Prompt entry

    Select Run as Administrator and accept the UAC prompt - the Elevated Command Prompt window should pop up.

     

     At the Command prompt, type

     CHKDSK C: /R

     and hit the Enter key.

     

     You will be told that the drive is locked,

     and the CHKDSK will run at he next boot - hit the Y key, and then reboot.

     The chkdsk will take a few hours depending on the size  of the drive, so be patient!

     After the CHKDSK has run, Windows should boot normally  (possibly after a second auto-reboot) - then run the SFC.

     

     SFC -System File Checker - Instructions

    Click on Start > All Programs > Accessories

    Right-click on the Command Prompt entry

    Select Run as Administrator and accept the UAC prompt - the Elevated Command Prompt window should pop up.

     At the Command prompt, type

     SFC /SCANNOW

     and hit the Enter key

     

     Wait for the scan to finish - make a note of any error messages - and then reboot.

     Post an MGADiag report with details of any error messages encountered.

    For whatever reason command prompt is not under the accessories tab. When I search for it in the "start search" field on the start button and press enter, command prompt comes up and says the following:

    Specified COMMAND search directory bad

    And then when i try to type the CHKDSK C:/R command, it says 'CHKDSK' is not recognized as an internal or external command, operable program, or batch file.

    I was never able to run command prompt as an administrator.

    OUCH - that sounds like you have some major problems.

    try this....

     

    Your best option in that case is to run the CHKDSK and SFC in offline mode from a Recovery Environment boot.

     

    Reboot the computer, and tap the F8 key until you get the advanced boot menu up - one option should be 'Repair your computer'. Pick that one.

     

    Log into your normal account.

    You'll get a set of options - pick the Command Prompt one.

    At the command prompt, type  DIR C:\

    - if we're lucky this will bring up a listing of your normal C: drive contents, including the Program Files folder(s) and the Windows folder.

    If not, try D:\ or E:\ (etc. until you get the right letter)

    then type the following command

     

    CHKDSK <drive>: /R

     

    and wait for it to complete - it could take a few hours, depending on the size of the drive.

     

    Once complete, type  

     

    sfc /scannow /OFFBOOTDIR=<drive>:\ /OFFWINDIR=<drive>:\Windows

     

    where <drive> is the letter you found above.

     

    Wait for the command to complete. (make a note of the response!).

     

    Once it has, type  EXIT and the pick the option to reboot.

     

    Post another MGADiag report

    So I was able to run the chkdsk in the recovery environment boot. When I first ran the chkdsk, it said "This type of file system is NTFS." Not sure if that means anything. At the end of the chkdsk, it said, "Failed to transfer logged messages to the event log with status 50."

    After running the sfc prompt, it said the following: 

    No offline repair is performed.

    For offline repair specify the location of the offline boot directory.

    For offline repair specify the location of the offline windows directory.

    e.g. sfc/SCANNOW

    /VERIFYFILE=c:\windows\system32\kernel32.dll

    I'm not sure if that tells you anything. It is still showing that my Windows must be reinstalled due to an unauthorized version. Here is the diagnostics report.

    PLease open an Elevated Command Prompt window and run the following commands - copy and paste the results to your reply

    DIR C:\Windows\ktmw32.* /S

    ICACLS C:\Windows\System32\ktmw32.* /S

    SFC /SCANFILE=C:\Windows\System32\ktmw32.dll

      Here are some instructions to make life easier :)

    1) To open an Elevated Command Prompt Window (the CP window), click on Start, All Programs, Accessories – then right-click on Command Prompt, and select Run as Administrator. Accept the UAC prompt. 

    2) To run the commands easier, highlight the block of commands, and right-click on the highlight – select Copy. In the CP Window, click on the black/white icon at top left – select Paste. The commands will run but may not complete the last command, so hit the Enter Key once. 

    3) To copy the results... click on the Black/White icon in the top left, and select Edit... 'Select All', and hit the Enter key - then use Ctrl+V or r-click+Paste to paste it into your response.

    As far as the command prompt goes, I still do not have access to it under Start>All Programs>Accessories. If that is a sign of a problem, I'm not sure. So what I had to do was do the F8 and "Repair Your Computer". I was able to run the scripts. After I ran the SFC line, it said there is a system repair pending which requires a reboot to complete. Restart Windows and run sfc again. So I did. I went back to the F8 screen and tried it again. It said the same thing. So then I reset the computer and allowed it to fully boot up. Still didn't have access to command prompt, so I did the F8 trick again. When I ran the SFC command, it gave me the same message. Here is the diagnostic report.

    As far as the command prompt goes, I still do not have access to it under Start>All Programs>Accessories. If that is a sign of a problem, I'm not sure. So what I had to do was do the F8 and "Repair Your Computer". I was able to run the scripts. After I ran the SFC line, it said there is a system repair pending which requires a reboot to complete. Restart Windows and run sfc again. So I did. I went back to the F8 screen and tried it again. It said the same thing. So then I reset the computer and allowed it to fully boot up. Still didn't have access to command prompt, so I did the F8 trick again. When I ran the SFC command, it gave me the same message. Here is the diagnostic report.

    You will not have been able to properly run SFC from teh Recovery environment, unless you used the OFFWINDIR and OFFBOOTDIR options

    Try this...

     

    Reboot the computer, and tap the F8 key until you get the advanced boot menu up - one option should be 'Repair your computer'. Pick that one.

     

    Log into your normal account.

    You'll get a set of options - pick the Command Prompt one.

    At the command prompt, type  DIR C:\

    - if we're lucky this will bring up a listing of your normal C: drive contents, including the Program Files folder(s) and the Windows folder.

    If not, try D:\ or E:\ (etc. until you get the right letter)

    then type the following command

     

    CHKDSK <drive>: /R

     

    and wait for it to complete - it could take a few hours, depending on the size of the drive.

     

    Once complete, type  

     

    sfc /scannow /OFFBOOTDIR=<drive>:\ /OFFWINDIR=<drive>:\Windows

     

    where <drive> is the letter you found above.

     

    Wait for the command to complete. (make a note of the response!).

     

    Once it has, type  EXIT and the pick the option to reboot.

    I ran the chkdsk from the F8 repair screen. When I did, I received a message that I want to make sure is ok to continue before I do. It said:

    Chkdsk cannot run because the volume is in use by another process. Chkdsk may run if this volume is dismounted first. ALL OPEN HANDLES TO THIS VOLUME WOULD BE INVALID. Would you like to force a dismount on this volume? (Y/N)

    I'm assuming this is from my previous attempt and that I should select yes, but I wanted to make sure first. Thanks.

    When you run SFC /SCANNOW from the F8 Repair, you MUST use the OFFBOOTDIR and OFFWINDIR switches - or you are simply checking the RAMDisk and not the installed system.

    I am following your instructions. I never got to the point to use the OFFBOOTDIR or the OFFWINDIR. As soon as I ran the CHKDSK as per your prior email, that's when it gave me the message that I was referring to. Should I say yes to it and continue?

    Per your instructions from 9/5/12, I tried running the commands. I rebooted my computer, tapped F8, and went into Command Prompt. I then typed DIR C:\    

    After that I tried doing the CHKDSK C:/R

    Per your next response on 9/5/12, I selected No and to start it at the next reboot. 

    I then rebooted the computer and let it completely boot up. During the boot up, a screen came up saying:

    Installing Service Pack: Stage 3 of 3. Do not turn off your computer.      It stayed at 0% for about 20 seconds, then showed 61% for about 10 seconds and then continued. 

    At that point, I rebooted the computer again to get into the Command Prompt screen from pressing F8. I tried running CHKDSK again with the same results. I then rebooted again, pressing F8. I skipped the CHKDSK prompt and went straight to the SFC prompt.  I ran the sfc /scannow /OFFBOOTDIR=C:\ /OFFWINDIR=C:\Windows

    The response showed the following:

    scf /scannnow

    scf /VERIFYFILE=c:\windows\\system32\kernel32.dll

    scf /SCANFILE=c:\windows\\system32\kernel32.dll /OFFBOOTDIR=d:\ /OFFWINDIR=d:\windows

    I suspec that you and I are talking at cross-purposes - you should NEVER have got the responses you say you did from running SFC properly - and if you didn't run it properly, then you couldn't have got those responses

    Try running SFC again now, while in normal mode. using this command

    SFC /SCANFILE=C:\Windows\System32\ktmw32.dll

    what response do you get?

    It appears that I am going in circles. I followed the directions of the article.

    1. Click Start, type Services.msc

    2. Double-click Windows Modules Installer

    3. Set its Startup type to Manual

    4. Click OK.

    Once it was set to manual, I then tried to run the command SFC /SCANFILE=C:\Windows\System32\ktmw32.dll

    In the command prompt screen, it says: There is a system repair pending which requires a reboot to complete. Restart Windows and run sfc again.

    So I did just that. I restarted Windows and tried to run sfc again. That gave me the error: Windows Resource Protection could not start the repair service.

    So then I opened up the Windows Module Installer as described in the article and the startup type was changed back to Automatic. I tried again to set it to Manual and run 

    SFC /SCANFILE=C:\Windows\System32\ktmw32.dll

    That took me back to the error: There is a system repair pending which requires a reboot to complete. Restart Windows and run sfc again.

    So I am lost running in circles. Any suggestions? Thanks.

    This possibly means that you have a 'stuck' pending.xml file, or that the SoftwareDistribution foler is corrupted.

    We can sidestep the problem for the moment by running SFC from the Repair Environment....

    hmmmm - let's try a different way to find out what's wrong (which i  specified before, but got sidetracked from).....

    Please open an Elevated COmmand Prompt, and run the following commands...

    DIR C:\Windows\ktmw32.* /S

    ICACLS C:\Windows\System32\ktmw32.*

    ATTRIB C:\Windows\System32\ktmw32.dll

    post the results.

    When I ran the command sfc /scannow /OFFBOOTDIR=c:\ /OFFWINDIR=c:\Windows from the repair environment again, it said:

    There is a system repair pending which requires a reboot to complete. Restart Windows and run sfc again.

    No matter how many times I restart, it says the same thing. I think the service pack is stuck and never finishes installation for whatever reason.

    That's something I was definitely not expecting!

    It looks to me as if something has replaced the file with an empty one, in both the normal position and in the backup in the winsxs folder.

    This smacks of enemy action (malware) or friendly fire (an AV misbehaving)

    We'll have to attempt to replace the file manually.

    I've zipped a copy of the file and uploaded it (ktmw32.zip) to my Skydrive at https://skydrive.live.com/#cid=936736BB8FCEB92F&id=936736BB8FCEB92F%21495

    Please download it and extract it to  C:\sprepair

    Then use the following method to put the files where they need to be

    1) reboot, and use F8 to access the advanced boot menu - pick Repair your Computer.

    Once you've logged in open the option for a Command Prompt.

    at the prompt type DIR C:\sprepair - if it finds the file, then great, if, not then try DIR D:\sprepair (and so on until you find it).

    Now use the following commands

     

    COPY <drive>:\sprepair\ktmw32.dll <drive>:\Windows\System32 /y

     

    COPY <drive>:\sprepair\ktmw32.dll <drive>:\Windows\winsxs\x86_microsoft-windows-transactionmanagerapi_31bf3856ad364e35_6.0.6001.18000_none_56d7500bd3f81b9d /y

     

    Change the <drive> to the proper drive letter you found above - there are only two commands- they will wrap so be careful!

    note that you MUST get those commands exactly right, or they simply will not work.

    Once you have both files in place, reboot to Windows normally

    Now run the following command

     

    DIR C:\Windows\System32\ktmw32.* /S

     

    I don't need the details of that - just the general look of it (is it still malformed in the same way?)

    now reboot again, and run another MGADiag report - post that.

    Files copied over correctly. As soon as I opened up the command prompt, it already was showing that it could not find the text. When I ran the script, it still looked malformed as before.

    OK - I think it's likely that your system is beyond realistic hopes of proper repair by manual means.

    I suggest that you back up all data to external storage, and attempt a repair install, following these guidelines...

    You may find this funny, strange, or just unexplainable but here's what I found. I came up to my computer to start backing everything up onto an external hard drive. I noticed that my computer must have restarted itself because it was left at my login screen even though I know for a fact that it was logged in when I left. So I logged in as normal. When everything loaded up, there was no popup that said Windows module stopped working. There was no mention in the lower right corner that this was an unauthorized copy of Windows. There was no screen telling me to activate now or later. In other words, everything appears as normal. I don't know if there is a way to check it or if it is even worth it. But something you did fixed it and I am extremely grateful for it. Thank you very much.

    Yeah, I'm not sure why it seems to be working now. The only thing I am having issues is that there are 17 critical update that I can't seem to load. It gives me an error 80070490. I tried downloading the updates from Microsoft and running them, but that didn't seem to help either.
